A forma epidemiologicamente mais relevante \u00e9 a diabetes tipo 2 (90 a 90% dos casos), cuja incid\u00eancia aumenta depois dos 40 anos, est\u00e1 relacionada a fatores gen\u00e9ticos e principalmente a problemas de h\u00e1bitos de vida (alimenta\u00e7\u00e3o inadequada e sedentarismo). A doen\u00e7a evolui se n\u00e3o tratada, resultando em desordens secund\u00e1rias como hipertens\u00e3o, nefropatias, problemas circulat\u00f3rios, retinopatias e outros problemas. <\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">Estima-se que cerca de 7% da popula\u00e7\u00e3o, em quase todos os pa\u00edses estudados, seja portadora da doen\u00e7a. Entre 1\/3 e 50% deste contingente n\u00e3o est\u00e1 diagnosticado e n\u00e3o tem consci\u00eancia da desordem. Nos Estados Unidos, acredita-se que o n\u00famero esteja em torno de 20,8 milh\u00f5es, com 6,2 milh\u00f5es n\u00e3o-diagnosticados. Estima-se que, nos Estados Unidos, 54 milh\u00f5es de pessoas t\u00eam \u201cpr\u00e9-diabetes\u201d (uma condi\u00e7\u00e3o que aumenta o risco de se desenvolver diabetes tipo 2, da qual eu sou portadora e fui diagnosticada aos 16 anos). <\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">No Brasil, o estudo conduzido entre 1986 e 1989 pela Sociedade Brasileira de Diabetes, CNPq e Minist\u00e9rio da Sa\u00fade mostrou uma preval\u00eancia de 7,6% da doen\u00e7a na popula\u00e7\u00e3o entre 30 e 69 anos. Metade delas n\u00e3o tinha conhecimento da doen\u00e7a. Outros estudos epidemiol\u00f3gicos n\u00e3o foram realizados por uma quest\u00e3o de custo (??), uma vez que este girou em torno de US$ 2,5 milh\u00f5es na \u00e9poca da primeira pesquisa. Acredita-se que a maior concentra\u00e7\u00e3o dos portadores esteja no Estado de S\u00e3o Paulo, com uma preval\u00eancia de 9,7% (<\/font><a href=\"http:\/\/www.diabetes.org.br\/imprensa\/estatisticas\/index.php\"><font face=\"Times New Roman\" size=\"4\">http:\/\/www.diabetes.org.br\/imprensa\/estatisticas\/index.php<\/font><\/a><font face=\"Times New Roman\"><font size=\"4\"> ).<o_p><\/o_p><\/font><\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">Os \u00f3rg\u00e3os p\u00fablicos concordam que o impacto da diabetes \u00e9 sub-notificado e sub-estimado. A American Diabetes Association afirma que a diabetes \u00e9 a quinta causa de morte nos Estados Unidos, sendo que 224,092 pessoas morreram de causas associadas a ela em 2002 (<\/font><a href=\"http:\/\/www.diabetes.org\/diabetes-statistics\/dangerous-toll.jsp\"><font face=\"Times New Roman\" size=\"4\">http:\/\/www.diabetes.org\/diabetes-statistics\/dangerous-toll.jsp<\/font><\/a><font face=\"Times New Roman\" size=\"4\"> ).<\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">Os custos da diabetes para a sa\u00fade p\u00fablica s\u00e3o gigantescos (<\/font><a href=\"http:\/\/www.pitt.edu\/~tjs\/diabecon.html\"><font face=\"Times New Roman\" size=\"4\">http:\/\/www.pitt.edu\/~tjs\/diabecon.html<\/font><\/a><font face=\"Times New Roman\" size=\"4\"> ). In\u00fameros estudos e programas de monitoramento indicam que estes custos t\u00eam apenas aumentado. <\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">\u00c9 consenso o fato de que a informa\u00e7\u00e3o sobre os riscos e medidas preventivas \u00e9 a chave para reduzir o sofrimento individual e custos p\u00fablicos associados a esta doen\u00e7a. A Organiza\u00e7\u00e3o Mundial de Sa\u00fade adotou um programa de orienta\u00e7\u00e3o para pol\u00edticas p\u00fablicas relacionadas a dieta e atividade f\u00edsica para enfrentar, entre outras coisas, a diabetes, considerada um dos resultados mais perigosos dos maus h\u00e1bitos alimentares e de atividade f\u00edsica contempor\u00e2neos (<\/font><a href=\"http:\/\/www.who.int\/gb\/ebwha\/pdf_files\/WHA57\/A57_R17-en.pdf\"><font face=\"Times New Roman\" size=\"4\">http:\/\/www.who.int\/gb\/ebwha\/pdf_files\/WHA57\/A57_R17-en.pdf<\/font><\/a><font face=\"Times New Roman\"><font size=\"4\"> ).<o_p><\/o_p><\/font><\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">A despeito dessa aparente consci\u00eancia quanto \u00e0 import\u00e2ncia da informa\u00e7\u00e3o e das mudan\u00e7as de h\u00e1bito entre portadores e grupos de risco para diabetes, o que impera nos materiais educacionais dos \u00f3rg\u00e3os p\u00fablicos e no discurso oficial da medicina \u00e9 o besteirol desatualizado. Uma falsa informa\u00e7\u00e3o como essa pode ser instrumental para o agravamento da desordem para um portador. Qualquer um que pesquise minimamente conceitos fisiol\u00f3gicos como resposta insulin\u00eamica, \u00edndice e carga glic\u00eamica dos alimentos e fisiologia do controle da glicose sabe que isso \u00e9 absolutamente falso: N\u00c3O, em hip\u00f3tese alguma o diab\u00e9tico pode comer qualquer coisa que o n\u00e3o-diab\u00e9tico come! A chave para o controle da RESIST\u00caNCIA A INSULINA, a base fisiol\u00f3gica da diabetes tipo 2, \u00e9 a produ\u00e7\u00e3o de um ambiente glic\u00eamico adequado aos tecidos endocrinologicamente ativos. Assim, n\u00e3o apenas a carga glic\u00eamica di\u00e1ria importa, mas o timing, a varia\u00e7\u00e3o de est\u00edmulos moduladores de resposta e demanda glic\u00eamica, mas o \u00edndice glic\u00eamico do que quer que seja administrado em doses absolutamente bem calculadas. Resumindo: a dieta do diab\u00e9tico, ou seja, a maneira como a demanda cal\u00f3rica di\u00e1ria de um indiv\u00edduo portador deve ser suprida, \u00e9 rigorosamente diferente daquela de um n\u00e3o-portador. Os m\u00fasculos \u201cfinos e compridos\u201d que o ignorante em quest\u00e3o menciona, s\u00e3o conhecidos tecnicamente como fibras tipo 1 e os exerc\u00edcios de baixa intensidade e alta freq\u00fc\u00eancia s\u00e3o, ao contr\u00e1rio do afirmado, os mais OXIDATIVOS e LIPOL\u00cdTICOS de todos, e n\u00e3o os mais GLICOL\u00cdTICOS!!! Em termos relativos, s\u00e3o os que mais consomem lip\u00eddeo, e n\u00e3o glicose, e em termos absolutos, s\u00e3o os que menos consomem qualquer substrato! Al\u00e9m de tudo, s\u00e3o os menores, de menor \u00e1rea de superf\u00edcie e com menor n\u00famero de receptores para glicose em termos absolutos! Eles proporcionam carboidratos, vitaminas, minerais e fibras. Amidos integrais s\u00e3o mais saud\u00e1veis porque t\u00eam mais vitaminas, minerais e fibras. Coma algum amido em todas as refei\u00e7\u00f5es. Comer amido \u00e9 saud\u00e1vel para todos, incluindo pessoas com diabetes\u201d<\/font><\/li>\n<\/ul>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\"><font size=\"4\">Errado!!! Essa \u00e9 a velha e j\u00e1 h\u00e1 muito descartada no\u00e7\u00e3o de que carboidratos \u201ccomplexos\u201d (amido, por exemplo), s\u00e3o menos nocivos do que simples (lactose ou frutose, por exemplo). Nada poderia ser mais falso, j\u00e1 que o que importa \u00e9 a convers\u00e3o do alimento ingerido em glicose e seu impacto na glicemia. Assim, o que importa s\u00e3o outros indicadores, como o \u00edndice e a carga glic\u00eamica. O \u00edndice glic\u00eamico de uma batata cozida \u00e9 de 111 e sua carga glic\u00eamica \u00e9 de 24,1, ou seja, uma das piores fontes poss\u00edveis de carboidrato para um diab\u00e9tico! O \u00edndice glic\u00eamico do p\u00e3o integral comum (n\u00e3o aquele especialmente elaborado para reduzir o IG, como os p\u00e3es quase sem farinha, tamb\u00e9m chamados de \u201cp\u00e3o alem\u00e3o\u201d) \u00e9 de 87 e sua carga glic\u00eamica \u00e9 de 16,5, n\u00e3o muito melhor! O jantar contem bolinho e frutas! O lanche da tarde contem algo que se assemelha fortemente a um sorvete!<\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">Mais para baixo na mesma p\u00e1gina desta insuper\u00e1vel figura, a famosa e tamb\u00e9m ultrapassad\u00edssima \u201cpir\u00e2mide alimentar\u201d, oferecendo ao pobre diab\u00e9tico a sugest\u00e3o de que se alimente predominantemente de amido como p\u00e3es e batatas (!!), frutas e vegetais na mesma propor\u00e7\u00e3o, quantidades bem menores de fontes animais de prote\u00edna e quantidades \u201climitadas\u201d de doces! Como \u201cquantidades limitadas\u201d?? A \u00fanica limita\u00e7\u00e3o poss\u00edvel a um diab\u00e9tico que um profissional com um m\u00ednimo de bom-senso deve sugerir \u00e9 a limita\u00e7\u00e3o absoluta!<\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">Passando para as fontes brasileiras, o besteirol fica na mesma linha. A primeira coisa que encontramos no site da Sociedade Brasileira de Diabetes (<\/font><a href=\"http:\/\/www.diabetes.org.br\/nutricao\/escalim.php\"><font face=\"Times New Roman\" size=\"4\">http:\/\/www.diabetes.org.br\/nutricao\/escalim.php<\/font><\/a><font face=\"Times New Roman\"><font size=\"4\"> ) \u00e9 a \u201cpir\u00e2mide alimentar\u201d, igualzinha \u00e0 bobagem comentada no item anterior. Para coroar, as seguintes frases:<o_p><\/o_p><\/font><\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">&#8211; \u201ccereais e tub\u00e9rculos \u2013 o amido deve ser a principal fonte de energia\u201d<\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">&#8211; \u201cn\u00e3o h\u00e1 frutas proibidas para quem tem diabetes\u201d<\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\"><font size=\"4\">&#8211; \u201co a\u00e7\u00facar, o mel e os doces podem prejudicar o controle do diabetes, se consumidos em excesso e fora do esquema alimentar\u201d<o_p><\/o_p><\/font><\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">Quanto \u00e0 atividade f\u00edsica, n\u00e3o h\u00e1 diferen\u00e7a dos \u00f3rg\u00e3os estrangeiros: a \u201catividade f\u00edsica ideal\u201d, para a SBD, deve \u201cser predominantemente aer\u00f3bica, ou seja, deve utilizar oxig\u00eanio em suas rea\u00e7\u00f5es e favorecer a queima de glicose e gordura para fornecimento de energia; Com base nessas informa\u00e7\u00f5es, as modalidades mais aconselh\u00e1veis s\u00e3o: caminhada, corrida, nata\u00e7\u00e3o, gin\u00e1stica aer\u00f3bica de baixo impacto, hidrogin\u00e1stica, ciclismo, dan\u00e7a etc.\u201d<\/font><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><o_p><font face=\"Times New Roman\" size=\"4\">&nbsp;<\/font><\/o_p><\/p>\n<p class=\"MsoNormal\" style=\"MARGIN: 0cm 0cm 0pt; TEXT-ALIGN: justify\"><font face=\"Times New Roman\" size=\"4\">Infelizmente, a comunidade cient\u00edfica mant\u00e9m uma atitude de bastante conservadorismo quanto a praticamente tudo que diz respeito \u00e0s estrat\u00e9gias de administra\u00e7\u00e3o de desordens ou condi\u00e7\u00f5es cr\u00f4nicas. Questionam dietas hiper-proteicas, atividade f\u00edsica intensa e tratamentos hormonais, tanto na diabetes como nas desordens mentais, envelhecimento e quase tudo onde a qualidade de vida esteja envolvida. Existem explica\u00e7\u00f5es sociol\u00f3gicas para esse conservadorismo, do ponto de vista de resistir ao aval \u00e0 evid\u00eancia emp\u00edrica que comece a emergir, n\u00e3o permitindo sua publica\u00e7\u00f5es em bons peri\u00f3dicos, e questionando aquela que consegue chegar l\u00e1. Quantos se sentem seguros para confrontar as prescri\u00e7\u00f5es de seus m\u00e9dicos, igualmente mal-informados? Quantos conseguem obter uma dieta detalhadamente calculada levando em considera\u00e7\u00e3o os par\u00e2metros relevantes? Este \u00faltimo aspecto \u00e9 mais cr\u00edtico ainda, j\u00e1 que dietas nesse n\u00edvel de detalhe s\u00f3 podem ser, na pr\u00e1tica, elaboradas com software adequado em tempo razo\u00e1vel. Eu mesma criei uma planilha e configurei as c\u00e9lulas para executar c\u00e1lculos baseados em par\u00e2metros que eu pudesse fornecer para compor a dieta de um portador de diabetes tipo 2. O tempo para criar menus \u00e9 t\u00e3o alto que passei a buscar softwares que me ajudassem.
